She has sex with other men whenever it pleases her.  I do not have sex with others.  That thing around my penis is a chastity device and it  serves many purposes one of which is to designate me as the cuckold and essentially remove me as her sex partner for the evening.  When she dates her Bull i greet Him at the door in my designated dress code which is naked and in chastity while she wears a little something for Him to remove.

That's a relief.  I know that some guys have penis piercing, which I assume stayed on during intercourse, but that thing (at least for me) would be too much.

Not that I know of.  Chastity belts seem to be a relic of the dark ages.  I have never seen one and suspect I never will.

Sometimes, but sometimes it stays on for another day or two.

Technically yes.  A cuckold can just be a man whose wife has sexual relations with other men.  But we like to add a bit of roleplay, servitude and even humiliation to the scene.
