1) I think that the camera resides in the "tented" part of a well excited vagina. I think you can also see the feed lines in some of the shots.

2) I saw something like this a long time ago on Discovery channel and I had always wanted to find it again. Thanks.

My guess is they used the pill-camera that was approved by the FDA in 2001. The camera transmits the images to a data recorder which is worn on a belt around the persons waist.
