Having been fortunate to have had the opportunity to closely examine many "artificially enhanced" breasts over the years, I can make a stab at answering your question.
Most that I have seen you could tell they were fake by looking at them, and for sure once you have had a chance to touch them. A few were so good you could hardly tell they weren't the real thing though. Of those, the one thing they almost all had in common is that they were the ones that were done under the muscle. Their have been a few that I have seen that weren't done under the muscle that looked pretty good, but all of them that I have seen so far you could definitely tell when you gave them a little squeeze.
Unfortunately, the majority of boob jobs I have seen weren't very good ones. They not only didn't look natural, but didn't feel natural either. That is mainly determined by how good the doctor that did them is. The only way to reliably determine which doctors are good at it and which are not, is to see the boobs he has done in the past, in person. Pictures mean nothing, by the way. If that isn't possible, then it is luck of the draw. Which probably explains why I have seen so many that really were not very well done.
Since I still refuse to discriminate between real and enhanced breasts, I vow to continue the research.
