She can orgasm with you because she TRUSTS you.. But to take that up another level or two requires something else.. a total relaxation of her mind and body to allow her to "go with it".. Some women we have known dont let themselves go for fear of many things.. the worst is being judged by others.. I am not a doctor but I could play one on TV.. the headaches could be a symptom of this, as well.. Consentrating too hard on "trying" instead of just letting it cum.. so to speak
