Vanilla is the term used to refer to those who aren't swingers (other alternative lifestyles use it as well to refer to those not in THAT lifestyle). So the line "Vanilla or...?" refers to the choice... you can be vanilla (which equates to bland) or ..... you can be something else (in this case a swinger).
