I've been shaving my balls and shaft and trimming "above" for several years. I get an annual physical -- including the testicular-tumor check and hernia check -- done by a GP. During the first few years I went to him for physicals I was not shaven down there, but since I started shaving he's never said anything or otherwise indicated any surprise at my ball-baldness.
I also get a full-body skin exam done annually by a dermatologist since I have fair skin and moles (and increased danger of skin cancer because I rarely used sunscreen in my youth), and I have some eczema and psoriasis. She's never said anything either, but I guess it's easier for her to see the skin without all the extra hair there.
Either way, unless your doctor is just out of med school, he or she has pretty much seen it all. But if your doctor asks you why you shaved there, just give a

and tell him or her you did it for
Lovinhim. And you can mention you were reluctant at first, but reassured it was safe to do and given recommendations on how to do it by a bunch of swingers you've never met in person.
