Some people get off on very dangerous sexual activities. This is one of them. Also, you have probably noticed that in general over the years adult videos push the envelope more and more. They constantly have to find new things to get the "shock value". People don't buy "vanilla porn" anymore so they keep finding new things to do to keep people interested.
Keep in mind that just because you see something a porn doesn't mean it should be done IRL. Most often the positions and techniques you see in a video are done for camera affect/angle and aren't even close to be comfortable much less pleasurable.
