For me, it felt exactly like you think having a sharp object forced through the hood of your clitoris feels, but it only hurt while being pierced. I'm not a wimp for pain (1 day of pain pills after breast augmentation, navel piercing, tatoos, etc) but I distinctly remember yelping. After that, surprisingly it wasn't sore or anything. It was definitely the fastest healing of any piercing I've had. As far as stimulation, I have a vertical and it does add to the sensation for me. My friend has a horizontal and she said it doesn't do anything at all. In any event, most people find it pretty sexy...I guess because it's novel.
