The difference is that one is over the pectorial muscle and one is under. Here's the specifics with illustrations, etc.

As far as the pros and cons:
1) Your implant may distort when you flex your pec muscles with unders
2) You may lose a little muscle strength with unders
3) If you have some sag, you may get double bubble with unders, which means you can see where the implant is, and then under that where your real breast tissue is
4) If you have little breast tissue, unders may hide ripples better
5) Overs have a lower rate of cancer detection from mammograms
6) Overs have a higher rate of capsular contracture (although, on some of the boards, most of the ladies that get cc have unders)
Yes, unders are more invasive and they say that unders have a longer and more painful recovery, but to me, being cut is being cut. I wouldn't say my recovery was a walk in the park.
As for me with my PS, unders wouldn't have cost any more than overs, but each surgeon is free to set his/her own rates, so your mileage may vary there.
